Okay, great. If you could arrive 10 minutes early to fill in the registration form, that would be great. You’ll also receive an SMS reminder to confirm your appointment.

Then I’ll just get your full name, a contact number, and your date of birth.

Okay, Annie, well, as it’s your first visit, I’ll get you to fill in this form. Do you have your Medicare card with you?
